ACE Test
The Angiotensin-Converting Enzyme (ACE) is an enzyme that converts Angiotensin I to Angiotensin II. Angiotensin II is an active vasoconstrictor that constricts blood vessels and increases blood pressure. The angiotensin-converting-enzyme test is a simple blood test that measures the levels of angiotensin in the blood. The test is usually ordered to diagnose sarcoidosis, a systemic granulomatous disease that mostly affects the lungs.
What is ACE?
When you have granulomas that produce minor bumps under the skin, a lingering cough, red watery eyes, and/or other symptoms suggestive of sarcoidosis; regularly when you have active sarcoidosis to monitor its course.
Why is the ACE test performed?
The test is useful for confirming sarcoidosis and is also helpful in the diagnosis and monitoring of the disease. The test may also help confirm Gaucher disease and leprosy.
- If you have signs or symptoms suggestive of sarcoidosis like chronic cough or shortness of breath, watery eyes, joint pains or granulomas,
- If you have been already diagnosed with sarcoidosis, this test may be ordered routinely to monitor the disease activity
When is the ACE test advised?
The test may be recommended if an individual has signs and symptoms suggestive of sarcoidosis, which include:
- Shortness of breath
- Chronic cough
- Red eyes
- Watery eyes
- Joint pain
The test may also be ordered for individuals undergoing treatment for Sarcoidosis to gauge the progress of the disease and to monitor the efficiency of the treatment in controlling the disease.
How is the sample collected?
A blood sample is collected from a vein in the arm.
How to prepare for the test?
Before undergoing the ACE test, the doctor or laboratory in charge might instruct the patient to restrain from eating and drinking for up to 12 hours before the test. Also, if the person is on steroid medication, it is better to ask the doctor whether they should be stopped.
ACE Test Interpretation
- An increased ACE level in a person with clinical findings suggestive of sarcoidosis means that it is likely that a person has an active case of sarcoidosis if other diseases have been ruled out. The finding of a high ACE level helps to confirm the diagnosis
- A normal ACE level cannot be used to rule out sarcoidosis since sarcoidosis may be present without an elevated ACE level
- It may also monitor the course of treatment in sarcoidosis. An initial high level that decreases over time usually shows spontaneous or therapy-induced remission and a favourable prognosis. A rising ACE level may show either the disease is not responding, or it is progressing
Are there any other conditions except sarcoidosis which can lead to increased or decreased ACE levels?
Decreased ACE levels may also be seen in lung diseases like emphysema, hypothyroidism, starvation, steroid therapy. The levels may be moderately increased in hyperthyroidism, diabetes, alcoholic cirrhosis, lymphoma, tuberculosis, leprosy.
